Job Description

Our Stoke team is looking for a genuinely enthusiastic nursing professional for an exciting Community/Rehab Nurse, based out of our Stoke branch on Main Road. We will provide you with the tools you need to succeed so that you can look after people the way they deserve to be taken care of – the Habit Health way.

In this role, you will deliver high-quality community nursing and occupational health services to a range of clients and contribute to smooth, efficient service delivery. This is an exciting and varied role where there will be opportunities for you to work on both contracts in social rehab, concussion and the pain space alongside our Occupational Health Service. Your nursing assessment skills, clinical judgement and proactive communication will help keep people healthy, safe and supported at work and in the community.

Due to the nature of the role, you will be required to travel around the region.

Main responsibilities:  

  • Provide on the ground support to our clients in the rehab space – both in the community and in the clinic. This could include rehab planning, wound care, associated documentation, client case reviews and discharge meetings
  • Work within relevant contract timeframes for service delivery
  • Deliver clinical occupational health assessments and provide clear feedback to clients 
  • Work with people from diverse backgrounds at different locations
  • Provide quality case assessments
  • Build strong professional relationships with clients, colleagues and external stakeholders
  • Promote Habit Health’s services and identify opportunities to support new and existing customers 
  • Maintain a good understanding of ACC’s contract operational guidelines and legislation

The ideal candidate:

  • Registered Nurse with a current NZ Annual Practising Certificate
  • Minimum 5 years' experience as a registered nurse
  • Excellent communication, organisation and time-management skills
  • Professional, proactive and solution-focused approach
  • Ability to work as part of a collaborative multidisciplinary team
  • Knowledge of relevant legislation (Privacy, H&S, health information)
  • Confidence using clinical systems and digital tools 
  • Full NZ Driver’s License
  • Able to compose and write succinctly and clearly and produce relevant reports
  • A people focused person who is warm and approachable
