Job Description

Pacific Health Group is seeking motivated and compassionate individuals to work as Rehabilitation Support Workers on our community rehabilitation contract. This is a hands-on role supporting clients to build independence in everyday life while working alongside a multidisciplinary clinical team.

We welcome applicants from a wide range of backgrounds including health professionals, support workers, employment consultants, vocational consultants, caregivers, and others with relevant rehabilitation or disability experience.

This is a flexible subcontract role, ideal for people looking to complement existing work or utilise their skills in a meaningful, community-based setting.

What You’ll Do

You’ll support clients in the community to work toward rehabilitation goals set by a Registered Occupational Therapist or Physiotherapist. You will not hold keyworker responsibility — your focus is delivering rehabilitation activities and sharing updates with the clinical team.

Support may include:

  • Supported Living – helping clients develop daily living skills, routines, and independence at home
  • Supported Activities – assisting with community participation and group programmes
  • Supported Employment – vocational support and work-readiness activities
  • Supported Transitions – supporting major life changes such as school-to-work or moving to independent living

Who Should Apply

We welcome people with experience or interest in rehabilitation and community support, including:

  • Health professionals such as Occupational Therapists, Physiotherapists, or allied health professionals
  • Support workers or caregivers with experience in disability, rehabilitation, or brain injury
  • Employment or vocational consultants
  • Students in health-related fields with relevant experience
  • Anyone with strong community support experience and a passion for helping people build independence

You’ll also bring:

  • Strong communication and relationship-building skills
  • Confidence working in community settings
  • A passion for rehabilitation, disability support, and empowering clients
